DJ Service Contract
I, JW of JW DJ Service has been requested by:________________________________________.
To provide my DJ Service for his or her event.The date and time of my requested service will be
On:_______, from ______( p.m.) til ________( p.m.,)available set time_________a.m./p.m.
I have agreed to provide my own equipment unless outside and a generator is needed,my
client shall provide me with a 110-120 volt current generator.The cost for my DJ Service for
your event will be
$______ __ hrs.There is a $50.00charge for each hour overtime.A non-refundable deposit
$100.00 is required within 10 days of receiving this Contract to reserve your date and time for
your event.After 10 days and no deposit has been received,your date and time will become
void.
The balance after your deposit has been receive will be $_______.The total balance due for my
DJ Service is due 10days before your event.You can also make payments at anytime and
online thru PayPal.Com. My payment email address is: JWDJSERVICE.COM
If for some reason your Event ends early beyond no fault of my DJ Service,you will still be
charged for the requested time of this contract.If for some reason your event has to end early
because of my DJ Service,such as equipment failure,the DJ becamed ill,and you have paid in
full,the remaining time of your event will be refunded to you within the next 5 working days.
You,the Client,shall furnish me with one 6 foot table to place my music and equipment on,
also at least one 110 volt outlet within 25 feet of my setup area.****My Client listed above
has read and fully understand and agree to the terms of this Contract by there forth
acknowledging with his or her signature below.
******This is a peak season event date but I have chosen to provide my Client listed above a
special discount rate.The regular rate for this event would have been this time of the year
$475.00 for 4 hours.
